To get you into our system, we need you to complete the form below and upload your completed Subcontractor Agreement,  W-9 and certificate of insurance (if you are insured). We have provided blank copies for you to print off below.

Your COI must include both general liability and workers' compensation, and have ACG listed as a certificate holder.

* all invoices needing payment on Fridays must be submitted before 3 PM on Wednesday. If you have any questions regarding our policy, please contact our office. 

 

** If you are located in the Shreveport, Little Rock, or Natchez markets, you will be paid via e-check.**
